Hospice/Palliative Care

Hospice care allows terminally ill residents and families to experience the end of life together offering peace, comfort and dignity.

Hospice care involves an interdisciplinary, compassionate team of professionals who provide nursing services, emotional and spiritual support for the terminally ill and assistance for families.  Hospice services concentrate on providing pain management, symptom control and comfort.

Bereavement services are offered to families to assist loved ones in coping with their grief.

Medicare benefits for Hospice services include:

  • Payment of medication, medical equipment and supplies related to the terminal illness
  • Scheduled Hospice nursing services
  • Scheduled Hospice nursing assistants for personal care
  • Social services, chaplain, volunteers, and music therapy

Eligibility – Physician certification of a terminal illness with life expectancy of six months or less for an illness such as:

  • Cancer
  • End stage:
    • Dementia/Alzheimer’s
    • Heart/Pulmonary disease
    • Liver/Renal disease
    • Diabetes
  • Stroke
  • General debility, progressive decline
  • Parkinson’s
  • Multiple Sclerosis
  • Increased dysphasia
