Admissions Checklist

  1. Contact our Admissions Director
  2. Bring the following:
    • Names, addresses & phone numbers of family contacts
    • Medicare Card
    • Supplemental Insurance Card
    • Social Security Card
    • Medicaid Resource Notice, if applicable
    • Advance Directives
      • Living Will
      • Durable Power of Attorney of Health Care
      • Power of Attorney
  3. If arriving from home, schedule appointment for:
    1. Medical history and physical exam within 5 days prior to admission
    2. Physician’s written orders for any medications or treatments required
    3. Nursing facility screen

Note:  A tuberculin skin test is required & will be done on day of admission.

What To Bring: Other Suggested Items:
Clothing/Personal Items (7 days worth) Books, magazines
Comfortable shoes/tennis shoes Family photos
Eyeglasses Items to make your room feel like home
Jacket/coat for outings Radio with headphones/batteries
Personal care products Small television
Pajamas, Robe and slippers Stationery, stamps, envelopes, pens
Sweater, washable  
Undergarments  
Wash and wear clothing  

Note:  Label ALL personal belongings (initial of first name and complete last name) with a permanent marker. 

Do Not Bring:
Wool items
Bar soap
Items required to be dry cleaned
Jewelry, money and credit cards
