- Bring the following:
- Names, addresses & phone numbers of family contacts
- Medicare Card
- Supplemental Insurance Card
- Social Security Card
- Medicaid Resource Notice, if applicable
- Advance Directives
- Living Will
- Durable Power of Attorney of Health Care
- Power of Attorney
- If arriving from home, schedule appointment for:
- Medical history and physical exam within 5 days prior to admission
- Physician’s written orders for any medications or treatments required
- Nursing facility screen
Note: A tuberculin skin test is required & will be done on day of admission.
